Phoenix Program

A leading Westchester Memory Care Neighborhood

Our Phoenix Program provides Alzheimer’s and Dementia care based on each individual’s special challenges in a secure memory care neighborhood on the 3rd Floor of Willow Towers Assisted Living facility. Using a creative approach, activities are designed specifically for cognitive and social stimulation.

Our Director of Health Care services, an RN with more than 20 years of caring for the geriatric population in a skilled nursing environment, now oversees and is the Case Manager of the health care program on our Phoenix Program.

We are proud to be partnering with White Plains Hospital Senior Services in a new pilot program entitled Partners in Dementia Care.  This program is a yearlong initiative of specialized enhanced education for our staff and caregivers. “Our goal is to continue our culture of excellence by giving our full staff the educational team building skills for our comprehensive collaborative model of care,” said Jean Dunphy, Executive Director of Willow Towers. The innovative program will give our staff special tools for understanding the perspective of a person with dementia, including Alzheimer’s disease.  Partnerships in Dementia Care Article PDF

The Phoenix neighborhood creates a sense of warmth and community with a:

The Willow Towers Phoenix Program monthly fee includes:

Daily assistance with bathing, dressing, grooming and incontinence
   management. Willow Towers nursing staff oversees and assists each
   resident with administration of their medications

Medication administered by Nurses with documentation on
   Medication Administration Records (MAR)

Compliance with physicians’ orders, monitoring of blood pressures as
   prescribed and communicating changes to our resident and/or the
   resident’s representative

A choice of a private or companion suite

All Housekeeping, personal laundry and linen services

Communication with each resident’s personal physician and family

The security of professionally trained and dedicated staff,
   24 hours a day.

Three delicious, nutritious meals daily, plus snacks

Individualized attention to health concerns.
